Physical Therapist

Position Summary

A Staff Physical Therapist evaluates, establishes a plan of care, and treats patients who are referred by a physician for physical rehabilitation services including: restoration of function, alleviation of pain, and the prevention of disability. 

Key Accountabilities

  • Evaluates, treats, and supervises the treatment of patients requiring physical therapy services.

  • Provides supervision of therapy treatment plans to allow for safe and appropriate progression.

  • Provides appropriate communication to referral sources to convey patient progress and mitigate concerns.

  • Assists in the completion of department quality assurance reports/records.

  • Appropriately utilizes and maintains department equipment to allow for optimal patient treatments.

  • Utilizes and requests supplies as needed to ensure department efficiency.

  • Provides supervision of Physical Therapy assistants in accordance with state regulations.

  • Supervises Rehab Technicians to promote effective guidance and communication.

  • Accurately completes all necessary patient documentation using current EMR including:  evaluations, daily notes, progress notes, re-certifications, and discharge notes in a timely manner according to facility, state, and federal regulations.

  • Maintains and completes individual timecard to ensure payroll accuracy.

  • Attends and actively participates in department monthly meetings to promote communication within the department and the facility.

  • Understands and complies with universal precautions and safety/risk policies/procedures to ensure individual and patient safety.

Minimum Qualifications

  • Doctor, Master, or Bachelor degree in Physical Therapy.

  • Licensed in the State of Nebraska.
